Some kind of device to keep the mouth open and to expose the surgical target is a requirement to perform transoral surgery (either robotic or some other kind). In September 2021 I felt compelled to write a post on that. It was a recurrent question from anyone starting a program. So the first answers are still there: you need a mouth-gag to star TORS. The classical blades of the traditional tonsillectomy mouth-gags, with a design to hold the endotracheal tube in the midline will interfere with the working space into the oral cavity. This is a particular problem for TORS in children.
