Overview

KAI is a microsurgical robotic platform developed by KouTech Medical Technology, a company focused specifically on robotic systems and instruments for micro- and supermicrosurgery.

The company describes KAI as a compact, multi-degree-of-freedom platform designed to address procedures requiring precision beyond the limits of the human hand. Its potential applications extend across reconstructive, vascular, lymphatic, maxillofacial, hand, ophthalmic, ENT and neurosurgery.

KAI is particularly interesting because it is being developed as a general-purpose microsurgical robot, rather than for a single specialty.

Company

KouTech’s corporate structure has an interesting international dimension. KouTech Medical Technology (Shanghai) Co., Ltd. was established in December 2022, with Kou Watanabe as founder and CEO. The company describes a global development structure involving research and development in Japan, clinical collaboration in China and a US R&D centre.

The company’s stated philosophy is:

“The limits of the hand should not be the limits of surgery.”

Its focus is on developing the robot, the microsurgical instruments and the underlying microfabrication technology as an integrated system.

Technology

The most striking feature of KAI is its very small wristed instrumentation. According to KouTech, the system can use 2-mm-diameter forceps with seven degrees of freedom at the tip. The company states that the platform is designed to perform anastomoses of blood vessels and lymphatic vessels below 0.3 mm in diameter.

The platform is also designed to be compact and rapidly deployable, multi-specialty and suitable for deep and narrow surgical fields.

KouTech emphasizes that it develops both the robotic system and its instruments internally, including its own microfabrication technology.

Regulatory status

KAI is not yet a commercially authorized microsurgical robot.

In May 2026, KouTech announced that KAI had passed China’s NMPA special review for innovative medical devices, entering the so-called Green Channel. The company described this as the first domestically developed general-purpose microsurgical robot to enter this pathway.

This should not be confused with NMPA marketing authorization: the green-channel designation is a regulatory-development milestone, not approval for commercial sale. As of August 2026, I find no evidence of CE or FDA authorization.
