Overview

DEXTER® is a soft-tissue surgical robotic system developed by Distalmotion, a Swiss medical-technology company headquartered in Lausanne. Its philosophy is somewhat different from the traditional robotic-surgery model. Rather than trying to reproduce the large, dedicated robotic operating room, DEXTER is designed around flexibility, mobility and integration with conventional laparoscopy.

The system has a particularly interesting proposition for outpatient surgery and ambulatory surgery centers: a compact footprint, rapid setup, a sterile surgeon console positioned close to the patient, and the ability to move between laparoscopic and robotic surgery during the same procedure.

DEXTER received its CE Mark in December 2020 and subsequently entered clinical use in Europe. In the United States, it received FDA De Novo authorization in October 2024 for adult laparoscopic inguinal hernia repair. Subsequent FDA clearances have expanded its US indications.

Distalmotion

Distalmotion was founded in 2012 in Switzerland with the objective of developing a different approach to robotic-assisted surgery: rather than making robotics more complex, make it easier to integrate into existing surgical practice. The company initially developed DEXTER around the idea of “robotic surgery without changing the way surgeons operate.”

This philosophy remains visible in the current system. DEXTER is designed to fit into existing operating rooms, use familiar laparoscopic access, and allow surgeons to switch between robotic and conventional laparoscopic instruments during a procedure.

The company’s current strategy is particularly focused on expanding robotic surgery into outpatient departments and ambulatory surgery centers, where the size, cost and operational requirements of conventional robotic platforms can be significant barriers.

Architecture

DEXTER is a telemanipulated soft-tissue surgical robot, but its architecture is deliberately compact.

Its principal components are a mobile robotic patient-side system with two robotic modules, an a sterile surgeon console. It offers an open architecture allowing integration with existing OR technologies.

One of the most distinctive aspects is the surgeon’s position. The DEXTER console is designed to remain close to the patient, rather than separating the surgeon from the operating table in a traditional remote console. This allows direct communication with the operating-room team and rapid access to the patient when required.  The system is designed to allow the surgeon to move between robotic and conventional laparoscopic techniques during the same procedure. That means that robotics does not necessarily have to be used for every step. This is fundamentally different from the traditional concept of a robotic procedure in which the robot becomes the central platform for the entire operation. It also explains the importance of the sterile console and the system’s small footprint.

Distalmotion has increasingly positioned DEXTER around the growth of ambulatory surgery. This is an important strategic distinction. Large robotic systems were initially developed primarily for major hospital operating rooms. Their size, installation requirements, dedicated infrastructure and cost can be less attractive for smaller facilities and high-volume outpatient procedures. DEXTER takes the opposite approach: make the robot sufficiently compact and flexible that it can be used where conventional laparoscopic surgery is already being performed. The company specifically targets procedures such as inguinal hernia repair, cholecystectomy and benign gynecological surgery, where a large proportion of procedures can be performed in outpatient settings.

Perspective

The principal question is whether flexibility translates into sufficient clinical and economic advantage.

DEXTER has a smaller footprint and a different workflow, but the system still requires robotic instruments, dedicated equipment and trained personnel. Its relatively compact architecture also means that it does not attempt to compete with the largest multi-arm platforms on every possible procedure.

Another important consideration is the current breadth of clinical evidence. Although European experience is growing and US regulatory indications are expanding, the accumulated evidence remains considerably smaller than that available for established platforms.

Dexter proposes moving robotic surgery beyond major tertiary hospitals and into outpatient surgery and ambulatory surgery centers. In that sense, DEXTER is not primarily trying to win by being a more sophisticated robot. It is trying to make robotic surgery easier to deploy.

Robotic Surgeon

Prior to commencing my venture into robotic surgery, I had established a firm grounding as an oncologic surgeon. My experience encompassed a wide range of resection and reconstructive procedures, as well as proficiency in minimally invasive surgical techniques and endoscopic surgery. These acquired skills and expertise serve as fundamental pillars for achieving success in robotic surgery.
